It is understood that coverage afforded under this contract applies only to the services included and to any wheel the is on the above described vehicle at the time of sale and the terms, conditions, price and limitations have been explained to me and are fully understood.

TERM AND CONDITION ON PLAN WHEEL INSURANCE COVERAGE
This contract, subject to terms, conditions and limitations as itemized on this document, is between Wheels Next and you, the original purchaser. Coverage on wheels expires (31 or 2, depending on the purchase plan) years from effective Date shown herein.

COVERAGE FOR PLAN

Wheels Next will repair or replace the damaged wheel for you. Destruction or damage to a wheel due to impact with a curb will be covered if the tire is not able to retain proper air pressure. There is no coverage for any other curb damage (scrapes scratches, chips or other cosmetic damages where the tire can hold proper air pressure).

LIMITS OF COVERAGE ON PLAN

1- All discontinued wheels will be reimbursed for damage on the wheel only.
2- Upon receipt of package you must mail or fax this insurange form to Wheels Next ATTN: Insurance Division. WHEELS NEXT, INC 2184 NW 89TH PLACE, Doral, FL 33172.
3- YOU MUST have an RA (return authorization number) from customer service prior to return the wheel.
4- Allow 15 days to process claim.

WHAT IS NOT COVERED (PLAN)

You are only entitled to the benefits as described under "Coverage and Limits". This contract does not apply to:
1- Any wheel damage, which is covered by the contract holder's primary insurance coverage or a vehicle service contact.
2- Replacement convered by warranty issued by the manufacturer of the wheels on the vehicle.
3- Unserviceable or irreparability or failure of loss due to negligence, abuse, misuse, or arising out of or related to a collision or upset, malicious mischief, vandalism, fire, chain damage, racing, theft, improper inflation pressure.
4- Off road use or use on a construction site. This DOES NOT constitute road hazards.
5- Destruction of a wheel due to improper tire pressure or less than 3/32 traeds wear.
6- Any consequential loss or damage whatsoever, including lost, damaged or injury to a person or property resulting from failure of any of the parts of the motor vehicle described herein, the replacement of which are covered under the terms os this contract.
7- All labor and shipping cost to and from WHEELS NEXT, INC.
